Scheduling your first appointment? Here are the basics:
Have ready for your call:
- Legal first and last name as shown on insurance card.
- Date of birth
- Social Security Number
- Phone number
- We can verify Oregon Health Plan & Open Card over the phone. (OHP assists available.)
- If you have other coverage, we can still help. It may take slightly more time to get approval.
- If self-paying, you will be advised of the costs over the phone.

- We can provide a screening over the phone in 10-15 minutes. We offer same/next day
appointments in most cases. - We treat the following Substance Use Disorders:
Alcohol, Cocaine (including crack), Heroin, Marijuana, Methamphetamine (and other amphetamines), Opioids and other prescription medications

- Legal first and last name as shown on insurance card.
- We will ask you some details about your drug of choice.
- If you are using opiates, we will talk to you about medication-assisted treatment.
- We will ask about your housing situation.
- We will talk to you about barriers that interfere with getting treatment, such as transportation and other needs you may have.
- We will schedule interpretation services if you need them.
- If you are pregnant and taking opiates, and you are seeking medication-assisted treatment, your best option will be walk-in service at our Portland Recovery Center (PRC) location at NE 11th Avenue and East Burnside Street. Priority will always be given to pregnant patients at PRC.
